CONSENT FOR THERAPY
From The Therapy Couch & Shreya Manot
By proceeding with therapy, you acknowledge voluntary participation in psychological services and agree to the terms below. A formal consent form will be shared prior to initiation of sessions.
NATURE OF SERVICES
We provide integrative psychotherapy tailored to individual needs. Feedback is encouraged throughout the process.
We do not provide crisis support outside scheduled sessions. Services are limited to 50-minute appointments.
APPOINTMENTS & PAYMENT
- Sessions must be prepaid, atleast two days prior for confirmation.
- Unpaid slots may be reassigned.
- Payment via UPI or bank transfer only.
- Booking and coordination occur via message outside sessions.
RESCHEDULING & CANCELLATION
- Free rescheduling only if requested >24 hours in advance.
- Changes within 24 hours are treated as cancellations with no refund.
- No-show (30+ minutes late without any prior information) results in cancellation which is fully chargeable.
- New booking and payment required for missed sessions.
- Therapist-initiated changes will be communicated in advance.
CONFIDENTIALITY
All sessions are confidential and privately maintained. Information is not disclosed without consent except where required by law or safety considerations.
Clinical notes may be maintained solely for professional use.
MENTAL HEALTHCARE ACT (LEGAL DISCLOSURE)
Confidentiality is protected under applicable mental health law; however, disclosure may occur under the following conditions:
- To nominated representatives involved in care decisions
- To other healthcare professionals for treatment purposes
- To prevent risk of harm to the client or others
- To prevent threat to life
- When required by court, board, or legal authority
- For public safety or security concerns
- To authorised internal administrative or supervisory staff for quality assurance
Only information necessary for the stated purpose will be shared.
MINORS
Clients under 18 require guardian consent. Parents may receive general treatment updates unless safety risks necessitate disclosure.
DUTY TO PROTECT
If there is reasonable risk of harm to self or others, confidentiality may be breached to involve emergency contacts, medical professionals, or authorities.
PROFESSIONAL BOUNDARIES
Therapy remains strictly professional. No personal, social, or financial relationships are permitted. Communication outside sessions is limited to scheduling.
TRANSFER OF CARE
If needed, clients may be transferred within the practice. Relevant notes may be shared internally for continuity. Private off-platform therapy with affiliated therapists is not permitted.
SESSION DURATION & FEES
Standard sessions are 50 minutes. Extended sessions (75 or 100 minutes) may be arranged and are separately chargeable.
CONSENT TO TREATMENT
By consenting, you agree to participate in therapy, understand risks and benefits, and retain the right to discontinue at any time. Therapy may be discontinued by the provider if safety or ethical boundaries are compromised.
RISKS & BENEFITS
Therapy may improve emotional wellbeing, relationships, and self-awareness, but outcomes are not guaranteed. It may also involve emotional discomfort and temporary distress as part of the process.
LIMITATION OF SERVICES
This is not an emergency or crisis service. Clients in crisis must seek immediate local emergency or medical support.
LEGAL LIMITATIONS
The provider does not participate in legal proceedings unless legally mandated. If compelled by court, standard professional charges apply. The practice is not liable for external actions beyond therapeutic control.
